Please complete the following form to register your child for Little Musicmaker classes.

Payments may be made via paypal, mailed or dropped off at the location where classes are held. Please note that your child's place in class is not reserved until payment is received.

Registration is ongoing until May 31, 2010. A two week notice must be emailed to billing@littlemusicmaker.com to withdraw your child from class.
